Can anybody give me anyadvice on creating a near miss reporting system? i am looking to implement a system soon and would appreciate any advice on how to make it as simple as possible to encourage more people to do it! Thanks very much!

Matt I think you are looking for the Holy Grail. You can make sure that near misses are included on your company incident report form, but that in itself will not necessarilly encourage near miss reporting. Some companies use reporting cards to make the process simpler or even phone lines. It really is a case of what will work for your employees, plus education about the benefits of incident (near miss) reporting. I am not convinced that the reporting of all near miss incidents does much to enhance safety, which I guess will partly depend on what you define as a near miss anyway. The Bird/Heinrich pyramid is allegorical and should not be taken too literally in my opinion.
