In the marine industry there is a government department for investigating accidents (along the lines of those in the rail and air industries) - www.maib.gov.uk They used to publish a bulletin about 3-4 times a year. I am advised that the last one due in August has not been published because a national newspaper managed to identify and publish the anonymised (spelling?) reports in previous bulletins. Accordingly we have lost a valuable resource owing to the appalling practices of tabloid journalism. I suspect that any similar safety alert by IOSH would run the same risk, regrettably.
