I am currently in the process of reviewing and updating my company's (pharmaceutical industry) current accident/incident reporting and investigation policy. I have been asked to undertake a benchmarking exercise to look at other companies existing policies/procedures and methods for reporting and recording incidents and investigations. This is where I need your help and I would really appreciate if a few people could send me some information so I can compare what arrangements other companies (not just in the pharmaceutical industry) have in place for accidents and incidents. Hi Kate Yes I am interested in looking at examples policies and procedures for different employers, how and whom investigates the incidents (i.e. are their different levels of investigation? and if so what does this consist of?) and what type of database or method do other companies adopt to record and anlysis their accidents (i.e. for trending) Claire
