Accident investigation and reporting software

This question has probably been been asked before but I am trying to source some software to effectively manage the accident investigation and reporting process. The process itself is fairly mature but, in order to broaden its scope and to provide more comprehensive reports, the administrative management of the expanded process may prove difficult. I would prefer a stand-alone system, as I am reluctant to pay for unnecessary bells and whistles but would like to have facilities for automatic and 'triggered' email contacts and updates. I work for a large company with in excess of 600 sites and more than 4500 staff and, by neccessity, approximately 60 staff involved in the investigation and reporting process - thus the need to effectively manage the administration of the tasks involved.

There is a company called EHASOFT who specialise in this kind of software, I am using it to manage all aspects of H&S and find it very easy to use, it will email all involved in A/I investigation and any C/A generated will be emailed to the responsible person and their superior.
You could contact them and they could give you access to the presentation site so you could get a better look for your self.
