Can you be more specific regardin the tasks/industry involved. Human factors is such a wide area - need to narrow it down a bit.

Guys, Thanks for your responses. The HSE has assigned me a task to "drill down" on Human Behaviour centered around HSG48. The task is enormous, as when I investigated 1 planned maintenance event e.g. shutdown of a vessel I ended up with 3 sheets of A3 resulting in what can go wrong if the wrong button is pushed. Help!!! am I looking too much into this? Cheers.

Yes, I think you're looking too much into it! What they want to see is essentially that you have identified (1) what critical task steps are there where human failure could result in a serious consequence - don't worry about the non-serious consequences (2) what are the types of human failure that are likely to be involved in each case (lapse, violation, etc) and (3) what can you do either to make these failures less likely (using the ideas of HSG48), or to make the consequence less severe.
