Hello Near miss reporting in the company is non-existent at the moment, I would like to start a campaign to operatives on the advantages of reporting Near Misses. Any advice on which road to go down to kick start this procedure. I was thinking about T-cards etc and posted anonymously or the incentive scheme. Any suggestions would be appreciated. Thanks

We ran a competition between departments over two months to encourage near misses and pro-active ideas with a league table. No prize but every entry was rewarded with £1 for a chosen charity. e got a few silly entries but te vaste majority were positive and we folowed every one up with an investigation or engineering action

At our company we run a series of cartoons where operators had to add a punch line the winners got £10. All of the cartoons were safety related. At the same time we launched a near miss hotline that operators could phone and leave details a near miss. All of the above were backed up by toolbox talks with operators in regards to near miss and accident reporting. All proved to be very successful.

We installed a new paper system where the person who initially raised a NM was given a copy of the whole investigation showing what action had been taken or why no action had been taken. This increased NM reporting by 80%, some were silly but most helped improve what was happening in the workplace.

Allan, have as many systems as you can - phone, t-card, e-mail, fag packet. The real driver for a near miss system is feedback for near misses raised and closing out issues. Tell your near miss reporter what is happening even if your not taking action because of x,y and z. Publicise your successes widely but remain sensitive to the contributor and don't embarrass them.

I agree with JohnJ. I tend to get the message across as a NEAR HIT rather than a near miss!
