I have been tasked with undertaking a work station assessment for an employee who is returning to work following injury through poor workstation set up. The workstation is a welding bench does anyone have a format they may wish to share with me???? or advice

I would look to employ a professional ergonomist or similar as this is specialist work

Here are practical guidelines for an ergonomic risk assessment:
a. Profile the task activities and goals – what the individual employee is expected to do, both physically and mentally, in some detail
b. Identify fully the classes of information required to carry out the tasks effectively
c. Measure of all relevant handtools, tools, equipment, furniture and systems he will be expected to use to carry out the tasks to the required standards
d. Measure all elevant physical ‘anthropometric’ measurements of the worker's body parts with attention to his age and physical or mental limitations – for legal compliance, relevant BS EN ISO standards as well as PUWER and other HSE regs should be cited
e. Sources of physical and mental hazards, which are likely to change over time.
While omissions at any stage are likely to result in pitfalls, the most common sources of error lie in failures of task analysis, especially failures to even recognise the host of ESSENTIAL ‘mental’ and verbal tasks often carried on without explicit awareness.
While I'm not sure what you mean by 'format', these guidelines have enabled me to become an ergonomist registered with the IEHF and the Central Register of European Ergonomists.

The colleague with whom I am developing 'Ergo Risk Manager' has agreed that I can let you know this information....
We're working on an Excel Add-in as a toolkit for ergonomic risk assessment.
In addition to the processes outlined above, it will include statistical techniques ( multiple regression models) to indicate returns on options for controlling risks of injury and of waste or loss. The 'waste or loss' is included so that it will be usable by managers responsible for production and quality. In time, the trade-offs negotiable between improvements in any of the three areas will be modelled.
It's taken time to work on the design as there appears to be no literature specifically devoted to 'ergonomic risk managment'.
