A member of staff recently sustained an injury at work, carried on with their shift and again for the next 2 days worked as normal. Then on the 3rd day they where signed off for 7 days by their doctor sighting the injury they sustained 3 days before as the reason for thier absence.
RIDDOR reportable?

RIDDOR - Yes!
You're clearly phased by the gap, however, could that be down to bruising, swelling, pain etc. not so apparent at the immediate time of the injury?

Yes. IMO.
RIDDOR doesn't say that its 3 days directly after an incident.

Defo RIDDOR reportable as had one where staff member sustained injury to hand but remained at work.
A week or so past and pain continued so went to GP who dressed wound.
Another week or so past and still really painful so back to GP who sent person to local hospital who said there had been damage sustained to the tendon and surgery was required to rectify.
Remained at work till appointment date and only then was the person sighed off (a few months now since the accident).
I contacted HSE when person told me she had attended hospital initially and was told reportable if signed off post operation (which the person was).
