At the weekend there was an accident at Ipswich Dock, where one man was killed and another seriously injured. They were loading a floating pontoon onto a lorry when it slipped. A man has now been charged with involuntary manslaughter. The best details can be seen at http://www.vertikal.net/en/news/story/13826/

Breaks your heart. I always look at lifts as procedural perfection, mainly due to the consequence of any failure. Obviously not on this occasion. So sad indeed.

Where does it say about involuntary manslaughter??? It is so sad

It has been reported on the BBC and ITV Local News programmes that a man from Clacton has been charged with involuntary manslaughter. The man who was killed was from Ipswich and the one who was seriously injured was from Lowestoft. In view of the comment from the crane web-site, using a crane without the outriggers is extremely dangerous, which would prompt such an unusual charge in such a short time.

This might be a good moment to remind forum users about 'live' court cases: "5. Discussion of potential or actual legal proceedings is prohibited on the public discussion forums. Public inquiries may be discussed, including where IOSH is a party to the Inquiry, on the Members’ Forum. Information that has been intentionally released, for example to alert others to a particular hazard or fact, can also be discussed in the IOSH Members’ Forum." Speculation as to the particular cause, etc, needs to be kept out of the forums, although comments on the particular facts as reported so far, as a warning to others, would generally be OK.

The web based crane site story and newspaper articles show a photograph taken at least 1 day after the event so they cannot be certain that outriggers or any other equipment was or wasn't used. It's a tragic event but I for 1 will await the facts first
