Folks I was wondering could anyone help me with the above. I am looking to redesign our accident investigation forms, injured person statement forms and witness statement forms. I have had a few complaints that the ones we use are missing information or duplicating information. I am also afraid that I will not have enough information available on the form. I also want to make sure I ask the right questions in the event that our solicitor has to defend a claim. Could anyone share copies of their forms for me to compare. Also in the event of a road traffic accident we have a different form , does anyone have a form that can do both an accident and an injury or could you send me a road accident form. Finally, a question. Do you see the normal HSE accident record form that we fill in first. Can we design our own so that we dont have to duplicate the same information on the above forms,. Eddie I would really appreciate your help.

Eddy, I attended an IOSH based course on accident investigation back a year or two ago. In this course it was suggested that we use the Police statement type forms for all interviews conducted following an accident. This was seen as quite easy to do and is admissible in any claims you may receive. They are flexible enough to allow the reporter to say exactly what they need to and don't pre-focus the type of answers therefore are quite free format and easy to get information the witness want to give. They are also easy for any court cases that follow. Getting company style forms filled in could be seen as a short cut and will not give the message of caring about an accident as it looks like a simple form filling exercise. Accident investigation needs to a very careful examination of all facts not just to answer questions on a form.

Eddy I have sent you template but it really comes down to how good the investigator is. You write the statement in your words that describes what the witness is saying, with the agreement of the witness by getting his signature to each page and amendment. NB The level of detail or extent of an investigation is determined by its seriousness or potential. Steve
