I have been asked to draw up a CDM Quality Assurance (Management) Document. Does any one out there happen to have any ideas? I do not simply want to copy someone elses as we have our own documents and procedures I have simply been tasked with bringing it all together and if anyone has some sort of template that would help, this would be useful.

That's quite a task to start from scratch. Competence is at the heart of succesful CDM compliance and a good place to start. You don't say the extent of your ORg's duty holder involvement, but the competency issue starts with the Client having a good appreciation of his duties, and applying the equally important part of ensuring comptency of other appointments.
Get that bit right and the QA process will be that much easier. Thereafter it's a matter of distilling the respective duty holder duties into a shape that suits your Org, and your other existing Procedures will help with the required shape. Ultimately you'll want something that is auditable. Sometimes it can be useful to start from the viewpoint of an Audit Process and work it back into a Procedure - inputs and outputs and all that.
Constructionskills.org.uk have some very good (free) duty holder-specific publications which complement the CDM ACoP.
It is possible to process-map CDM projects, but the mapping will be very specific to your own Org.
