Dear all, I am looking for a simple but effective system for reporting near misses. I do not wish to offer incentives e.g. vouchers or gifts etc. I would like to expect each employee to look after each other and ensure their colleagues and themselves go home safely. I appreciate the reluctance of some employees not to report near misses as they may fear repecussions for doing something they should not have been doing. I would like to throw this open to other members for their comments and to offer any positive suggestions or ideas in how they overcome a similar situation. Thanks in advance Regards, Fredg

Fred, as far as I'm concerned all incidents are reportable and near misses are incidents. End of discussion. There will always be the risk of staff expecting blame but that's for their managers to dispel. Can't get any worse than not having any of them reported.

Hi ya, Its all about the team of guys you have, and I personally find talking to the guys really helps, they may not feel like they can just walk to the office as their foreman might give out, so on my site walk(s), I make it a point of going talking to each gang and having a chat, more often then not they will always mention something, be it something they seen, or something they are not happy with, and I thank them for telling me whatever the issue was and remind them that if they dont tell me nothing can be done to rectify the issue.

Fred, it doesn't matter how they get reported, I'll take them from the back of a fag packet as long as they are reported. The real driver for near miss reporting is that some action occurs, even if its just to say we're not doing anything with this at the moment but thanks for highlighting it'.

We use the same reporting system as the incident reporting system. It’s electronic and anybody in the organisation can use it to contact H&S directly. We get back to the reportee and tell them something, even if it just as John, says,' thanks for the info.'

I developed an easy to use system whereby anyone could text in on their mobiles and it was logged on my PC (Vodafone helped create such a system)- this dramatically increased the number of reports
