As asked what sort of audit do you ant in what sort of an area? There is no standard template. I don’t use a form anymore as I have been auditing my site for 10 years and I know what is going and what to look out for. The nature of the audit has changed as well. 10 years ago it was more of an inspection, asking if everything was working, staff using PPE and no obvious hazards. Then we went over to looking at systems in place eg risk assessments done and upto date, staff training records complete etc. Now I am looking at the culture, how engaged the staff and management are when it comes to H&S. So what do you want?

Thanks to those who responded. Its probably more of a workplace inspection I'm looking for. I had a supplier audit last week and they insist on this being done and recorded. I already do this as a matter of course however I know there will be a wee form sitting on somebodys laptop which I could use covering all the usual stuff like fire, first aid, warehousing, workplace transport and so on >>>>>>>>>>>
Thanks again

Hi Jim, best thing is make up your own one specific to your sector/ trade/ workplace.
I have a template, which consists of boxes at the top of the page which are meant to jog the memory on what to look out for, such as welfare, notices, insurances, lighting, access/ egress, training needs, fire arrangements, PPE, PAT Testing, housekeeping, plant, equipment, lifting gear/ accessories, noise and vibration, abrasive wheels, permits, waste management etc.....then you can either tick the box if everything is ok or put numbers against the points above and then go onto the report section and expand on that topic.

You could probably tailor something based around the core elements of the Workplace regs/six pack. I have to say though that this sort of request sort of undermines what proportionate and real time safety management is supposed to be about. Simply 'lifting; a tick box sheet from somewhere else doesn't give confidence that you've done any intrusive focus on your own activities.
I dont mean to be rude like, but confusing audit with inspection is a bit worrying anarl!
