Hi All, I am setting up a OHSAS management system and am introducing and incident investigation procedure, only 28 employees, and our moto is KISS(keep it simple,stupid) Has anyone ideas or templates i could use ?

Steve, May I suggest downloading HSG 245 "Investigating Accidents and Incidents". It has some useful guidance and templates (including some worked examples). Hope this helps. PH2

Stupid? Well, any suggestion of stupidity would not go down well in our workforce. Simple? If only it always was simple. Many accident investigations are NOT simple, like when nobody seems to know what happened, or if you suspect the injured person is not telling the truth. Good luck with that! :o)
