Hi - does anyone have or know of a good example of an accident map, typical for a large factory?

Do you mean a map for layout of a large factory for recording the process of an accident?
There is an app for iPad that can draw a building layout, it is called MagicPlan, and can give you option for domestic of commercial plans. It is very good, although can take a little time to get you use well.

Paul,
All I can suggest is that you get the Engineering Dept to run you off a site/workshop plan or drawing. Then get hold of a pack of coloured dots, use different colours for different injuries, times of day etc etc.
On the KISS principle it would be easy to set up and get running and would illustrate whether there were any patterns OR show there wasn't much point in spending too much time on anything more fancy!

Paul,
Bit confused as to what you are looking for here? Mapping as in process mapping or a document map wouldn't normally show the information you are looking for. What do you want to do? Is it for compiling monthly accident data?
