Ladies / Gents a moment if you please, my QSHE department are telling me that the reporting procedure for a first aid on night work here is electronic, I say it is the book form and then electronic in the morning when someone in the morning knows how to get into system. My employers are getting quite uptight on my defense, I understand that the system should be put into operation as soon as a first aider takes on the IP, and the book should be placed by the first aid post. What happens if the electronic version fails and the day shift know nothing of it ? What happens if the IP takes ill over the w/e and no one knows of it for several days? I need some help

duty logbook or diary should be completed to enable transparity

What does it say in the company policy, The duty first aider usually tells the IP, to fill out the accident book after treatment, if they cant write then the first aider usually does it, there is no given time limit to say, when to fill in the accident book. I would keep a log in the first aid room, of accidents so there is no confusion, when it comes to a claim. The IP can insist that the accident book is filled in at the time of the accident, which is their right as an employee

I don't think the Social Security (Claims and Payments) Regs specify a time period in which to record accidents, just uses the phrase that the reporting means either paper or electronic is 'readily available'. I'm with Terry in that it depends on what it says in your policy/procedures. This is likely to be an operational issue rather than a legal one. I think you have good cause to argue that the means of communicating accidents between shifts is important in relation to investigation and what needs to happen to prevent a recurrence it may even warrant a review of a risk assessment, as well as the points you raise about failure of electronic versions and prolonged absence.

Is any system perfect? Books get lost or misplaced so it no guarantee just because its in a book that someone could find it. If the IP took ill over the weekend what difference would it make to people knowing about it if it were electronic or paper form? Brian

I don't think it actually matter much whether it is paper or electronic - the important thing is that whatever your policy says the means of recording must be available at all times. WRT recording this obviously needs to be confidential and as such there is no need to share the recorded details with the "day shift" - what would need to be shared comes later - post investigation when and if there needs to be changes made to procedures etc to prevent a recurrence. Unless I have got the wrong end of the stick!
