An error provocative environment is an environment where the lack of controls and poor culture produce errors which in a H&S context, will manifest as unsafe acts.
i.e. the environment provokes (i.e. invites) error.

I've never heard this term but it sounds similar to "situational violations". Examples of errors, their causes and their prevention are in HSG48 (www.hse.gov.uk/pubns/books/hsg48.htm) and there's more here: www.hse.gov.uk/humanfactors

I think this may be an example
The roads of Milan or in Egypt are good examples of provocative environment, where the lack of enforcement of basic road law and cultural perception of risk lead to unsafe acts. The behaviour of those, who in a correctly controlled environment would be compliant, changes in order to make any progress on their journey.
