If a contractor (small company with 3-4 people) has an accident on a refurbishment project that falls under CDM, who's duty is it to report an incident under RIDDOR, the contractor or the principal contractor?

The duty rests with the employer. CDM context is irrelevant.

Unless it is a non-employee who gets injured. Have a look at Table 2 in the HSE Guide to RIDDOR (L73) - the person "in control of the premises " is responsible for reporting injuries to the self-employed or persons not at work. In CDM terms - this could be the Principal Contractor.
