Also before putting in the time and effort, I'd make sure that management are thoroughly on board; once spent weeks working on an HSE Climate Survey (don't know if it's still available) years ago only for management to fail to take action based on the answers they got.

A well-designed climate survey is like any tool, something to use to deal with (fairly complex and emotionally alive) reality. Unless your management are committed to actively using answers given by even the best-designed survey, its use achieves nothing AT BEST
