Does anyone know of Human Factors training courses/modules, preferably interactive, based on HSG48 i.e. understanding what are human factors, types of errors and influences on behaviours?

A number of organisations run courses, although I think many of them are most concerned with major hazard industries. They include:
* ABB
* Human Reliability Associates
* Sheffield University (module on process safety MSc)
* Lancaster University (also, module on MSc)
* Institution of Chemical Engineers

As HSG48 is a (good) small distillation of an enormous amount of research, 'courses/modules' generally present detailed research and/or share working practice about very specialised applications for human factors rather than the much more basic issues you refer to.
In terms of the media you refer to, you may find something of interest on the websites of Technical groups within the (American) Human Factors and Ergonomics Society, www.hfes.org. e.g. 'Cognitive Engineering and Decision-making, Augmened Cognition. Human Performance Modeling.
In relation to the fairly basic - yet complex, interesting and far-reaching - areas of understanding you refer to, the following titles provide a guide to relevant research:
1. Human Error. J Reason, CUP, 1990
2. The Psychology of Safety, E Scott Geller. CRC Press.
3. Ergonomics at Work. Human Factors in Design and Development D J Oborne. John Wiley & Sons. 3rd edition. 1995.
4. Human Performance: cognition, stress and individual differences. G Mattthews, D R Davies, S J Westerman, R B Stammers. Psychology Press. 2000
The Clinical Human Factors Group organise workshops which may also offer relevant insights to a growing range of applications in medical treatment and healthcare.
