Hi, I am currently half way through my NVQ Level 5 diploma and need to conduct an Event & casual factor analysis and don't know where to start. Does anyone have one that I could have a look at to point me in the right direction? Thanks

Rob Surely there is an example in your course work? I think you need to be a bit more specific. not sure you looking for an investigation report, or just a basic metaphor describing a process such as root cause analysis? Ray

The EFCA tool (under its current name) is mentioned in the free IOSH publication 'Learning the lessons', plus a website detailed in ref. 17 of that document. Hope this helps.

Some good points above, is this about determining root causes by using a tool such as cause tree analysis etc? However, there is a US system called "causation factors" which goes about it from the opposite direction - there are list of hazrads etc which you go through and rule out those not relevant, until you get to the root causes eg http://www.gobookee.net/...rch.php?q=causal+factors My take on this is that the question is about how to determine root causes; there are many ways to do this. Causal factors and root causes are really the same thing. Hope this helps.
