Health Questionaires for short term contracts

We frequently employ some staff on short term contracts. Obviously we need to know if they have any pre-existing medical condition eg; diabetes. It would never stop us employing people but we need to know so we can manage the member of staff correctly.
My question/request is: does anyone have a basic form/questionaire I could crib from please.

I agree with Ron on the need to be very careful about asking qustions. E.g. why would you need to specifically as about diabetes. If somebody has diabetes that is well controlled and stable, and it is unlikely to ever affect their work, asking questions at the pre-employment stage could well fall foul of the Equality Act.
The only question that needs to be asked at the pre-employment stage is "do you have any medical condition that might affect your ability to do the job for which you are being engaged? We ask this question purely so that we can assess whether we need to make any reasonable adjustments".
If the answer is no, no further action is required unless it transpires that the person has lied. If the answer is yes, referral to an Occ Health Professional is required.

We use pre-employment health screening carried out by occ. health, after a conditional job
offer has been made and before they start work, and they issue a fit
note. If anything like diabetes is revealed occ. health can ask the candidate for access to
medical records held by their doctor. If the doctor confirms that the diabetes is well
controlled then occ. health will issue a fit note.

This is NOT a pre-employment questionnaire, as some people are assuming. It has no bearing on a persons ability to do the job we are asking of them. All we, as responsible employers, are requesting of employees is that they inform us of any chronic condition so that we can assist the staying healthy and at work. Obviously it is in our interest to keep staff well.

Amongst other questions, we ask the following information on a 'Emergency Information Form' which includes next of kin details and GP details and is requested as part of the induction process:
1. Do you suffer from any long-term illness or allergy including diabetes, epilepsy, asthma or heart problems? If yes please give details.
2. If yes to above, do you give us permission to inform first aiders on site?
At the top of the form we state "Medical Information in needed to ensure your health, safety and welfare, and is kept completely confidential. No information on this form will be divulged without your express written consent except in the event of an emergency where it may be disclosed to relevant emergency services personnel."
