Hi

Sorry if this is so obvious I can't see it, but if you have a fatality to a member of the public that is work related do you report this to the HSE by telephone or via the internet?

Please keep all replies professional.

The above is from http://www.hse.gov.uk/pubns/indg453.pdf. I think the use of but is ambiguous as quite a bit of the phrasing in HSE RIDDOR publications.

Personally I would make a phone call but then follow it up by also reporting on the internet.
Sounds belt and braces, I know, but that way you have a paper trail.

By phone first - they have a 24 hour emergency number followed up on line

Many thanks for all your replies.

My thoughts were that I would call first and give a brief explanation (being careful not to drop myself in it) then follow up with an online report and wait for the HSE to arrive.

Do they still attend all fatal accidents at work including those to MOP?

If my memory serves me correctly, you don't Normally need to follow up on the phone call as they will take all of the details and give you the reference number.

I accept that an 'out of hours' report may differ

Have reported a number of fatalities all by phone as need to ask Inspector whether the area is to be cordoned off and not touched or whether you can clear up (as in hospital environment may need to still have the area operational).

HSE will inform the Police where required as per the protocols signed for information sharing them (I have never reported a work place fatality directly to the police).

The last fatality occurred out of hours (over a christmas period) and rang the HSE and was put through to someone on the other side of the country who then contacted a local inspector who then rang me back within 20 mins or so.

In that case the inspector did not come to the site but in the current climate of FFI would envisage that there is more a likelihood of them coming (at a cost of £124 per hour oer inspector)!
