I don't have it any longer - but I did have just this in a spreadsheet format, and it was very useful. One column with the questions (about the weight, the lifting distance and so on), a second with the guidance on what counts as high, medium or low in each case, a column for the ratings (automatically coloured red if you enter high, and so on) and a column for comments to explain how the ratings were arrived at.
