I did an accident investigation course (in house with Carillion) a numbe rof years ago and they used a really good scenario which involved an "accident" in a supermarket car park with broken glass etc. The deleagtes read the description and answered questions but the point was that the accident involved someone dropping a glass bottle - the aim being to stop people making assumptions but to find the evidence.

I would love to use this if anyone has a copy
