When using the F10 as CDM C there is a choice of three boxes to tick at the end of the form.
I am notifying the project as CDM C and usually tick the "on behalf of the client" box then enter the name of the client's Project Manager.
This has been criticised on a site audit as not being correct.
What do you think?
The emailed F10 then comes to me as I have entered my email addy in the CDM C information, and I issue it for the PC.

As you are the CDMC and are notifying it should be your name you enter after ticking the box.
You are, for all intents and purposes, signing this on behalf of your client to verify they are aware of their duties.
If you put someone elses name in this space it could be read as they are the ones doing the verifying.

All projects are to be notified by the CDM C.
That is one of the CDM Cs duties.
Why then are there three alternatives for signing the HSE extranet electronic notification form?

Self explanatory really.
Box 1. As client I am aware of my duties ( tick if you are the client and have not appointed CDMC as you become CDMC by default and are aware of this and your other duties).
Box 2. As CDMC I declare the client is aware of their duties (Tick if you have an experienced client who you have worked with before and you know they are aware of their duties).
Box 3. As CDMC I have made the client aware of their duties ( Tick if you have an inexperienced client and you have fulfilled your duties as a CDMC by explaining to them their duties as a client under the regulations.

Nice concise answer.
Do organisations get caught not completing F10 or appointing A CDMC anyway (unless someone gets hurt). I can't imagine [reference removed] drive around with a list of who has or has not done the above? I can't imagine Joe blogs can get access to a list to see if a building site near them has done what they currently should. Interesting the form allows for experienced and not experienced clients.
Not in the construction game myself, so always wondered.

When I worked in an HSE Construction Group many years ago, we each had our own patch.
When the old Forms 10 came in from Main Contractor they went to our Principal Contractor who would mark them top right hand corner.
NV - No visit - filed
PV - Possibly visit - copied to Inspector in patch - copy to file
V - Visit - as PV
Very few marked V - more than 6 weeks duration high risk works such as demolition, steel erection, roofing.
So yes, when going round patch had a folder with loads of Forms 10.
Not all marked V got a visit. Principal Inspector instruction no to plan work by F10s as at least the ones who submitted had some understanding of requirements and compliance.
Plenty of other notifiable projects where contractor didn't know (or pretended not to know) about notification requirements, or simply just didn't want to attract HSE attention.

I never thought Health and Safety Executive actually drove around with a list, would have thought that was mind boggling, depending on your patch. (not sure why my earlier reference to Health and Safety Executive - HSE was removed).
It only came to mind as the other day I drove past a refurbishment of a building that had a notice on the front stating it was under CDM, which made me wonder. I bet there are lots that get away with it though.
