A charity I do some work for has asked me to design the above mentioned form. They would use it to send to employers who may be willing to offer (and give) a job to adults with learning difficulties. I would see the following type of questions: Perspective employer's details and H&S Standards (Policy, Insurance details etc) Appropriate risk assessments in place. Is 1st aid available Supervision, training and instruction for client PPE available Fire and Emergencies Heating, lighting, ventilation etc Key health and safety considerations to be consider by the employer. Can anyone please add to this list - your help appreciated. Rich

Maybe look to recognise a couple of pre-approval schemes under the banner of SSIP? Depending what sectors you are looking at you could recognise the relevant scheme - construction line,CHAS or similar... Just a suggestion..

You may put off a prospective placement provider if you request all that detail up-front, and little of what you're asking for would help with assurance that the employer could effectively manage someone with learning difficulties. Beyond insurance details it may be of greater benefit to request a narrative response based on information provided by the Charity about the potential range of learning difficulties the clients have. Key elements you're looking for are how they'd approach supervision, instruction, communication of information (including emergency information).

I have assumed that this check would be made after an employer has indicated a willingness to work with the charity? So Ron's comment above is very relevant. Recall the recent hoo-haa about work experience checks on employers and tick list evaluations and H&S assessments? This sort of employer pre-assessment is really 'out of fashion' at the moment re such specific/special situations. What is more appropriate than a tick list assessment is a dialogue between the charity and provider to explore s&s risk controls given the special needs of this group of people. Information from the charity re specific needs; employer knows risk controls of their workplace; work positively with the employer to agree risk controls, start placement. Have a site visit by all means to check physical conditions to give some re-assurance or if you think it relevant. Simples! If the employer can't hack that approach then that is much better indicator, of their inability to properly manage employees with special needs, than any amount of tick list paperwork. Otherwise if you decide to go ahead with a pre-assessment. The list headers that I used until recent changes were (add your own questions along the lines you indicate): (apologies for caps, it is a cut and paste) --INSURANCE --H&S POLICY & ARRANGEMENTS --INFORMATION, INSTRUCTION, TRAINING & SUPERVISION ) --EMERGENCY ARRANGEMENTS --ACCIDENTS. --RISK ASSESSMENT. --WELFARE. (inc PPE) --MAINTENANCE & INSPECTION OF MACHINERY/EQUIPMENT/ELECTRICS --SAFEGUARDING. --DEVELOPMENT ISSUES (things the employer agrees to improve) --REMEDIAL ACTIONS (things that have to be done before the placement can start)
