I am not sure what you mean by reactive v proactive when it comes to monitoring/auditing.
Personally, I draw up a list of things/ area that (based on previous experience) require monitoring and then I arrange to monitor them. Is this proactive (I don’t wait for someone to tell me what needs monitoring) or is it reactive (since it is based on previous data like recent accidents)?

Reactive monitoring uses things like incident reports, maintenance records, failure reports and so on; it's reactive bacuse you will be reacting to things that have already happened.

Active monitoring means checking on things which are happening at the time; Audit is usually a form of active monitoring. You might be looking at temperatures of calorimeters, presence and use of guards and so on.

HSG 65 (old style) has some useful info about this,

Refer to Examples 7, 8 & 9 on pages 120 & 121 of BS18004:2008; "Guide to achieving effective occupational health and safety performance"

It has 23 leading indicator examples, 10 lagging indicator examples and 5 that can be either leading or lagging. Pls PM me.

Actually, as a reference, especially the Informative Annexes A through to L are most useful. It is a fairly expensive standard to buy, but probably worth it or it will be a part of the Barbour/IHS subscription.

(I was involved in BSI's HS/1 committee from 1998 to 2004 when this was BS 8800:1996 & BS 8800:2004)

I am glad it was so "easy" for you :-) I was not requesting the information to learn, it was for a presentation to make a reference to.........Thanks anyway Steve lol
