I am interested in discovering whether there is a general perception that mushroom caps for rebar safety is acceptable or whether we feel that mushrooms are not fit for purpose and construction sites should be more pro active when it comes to insisting on a anti impalement alternative?

If they are being used as just general protection from sharp edges of rebar etc then they have their place. If they are being used as anti impalement protection then they are clearly not suitable (unless of course they are the correct re-inforced ones).

I agree with paulw71: these should not be used to prevent impalement. OHSA published guidance about these:
