Useful stuff to remember / do / take when investigating an incident

So, one line answers only, what is not mentioned in course notes, policies, LP’s etc, that you wish was, I’ll start with;
Take a spare pen in case the first one runs out mid investigation or interview.

Two suggestions:
1) Look, but don't touch until you have to
2) Take lots of photographs BEFORE you start moving things around

make sure the area is safe to enter
make a list of names of witnesses
take serial numbers and model numbers of equipment involved
collect any relevant documents (training records, daily check sheets and cleaning records etc)

Where possible have a dictation device to record statements or record what exactly you have done

I usually have a ruler too. It came in useful when photographing a hole in the floor prior to it being repaired.

Chalk to mark floors and walls to highlight things.
Lie detector ( or box with small flashing red LED that has lie detector written on it).
Binoculars ( or camera with large optical zoom)
Look at other thread about what to take in an audit a few weeks ago.

Take more photos, from different angles, including from above or below if appropriate.

My first question would be: "What sort of incident am I investigating?"
Could be a major fire, explosion, fall from height, FLT overturning, chemical spillage, skin burn, etc., etc. In my experience each would require a different set of equipment. Of course, there would be some items that would go to each, but some might require special equipment.
First thought: "What PPE should I take?"

Always keep asking why? Why did they get hurt? Why wasn't the guard in place? Why was it broken? Why wasn't it reported or maintained? so on and so forth. It can in some cases, get you where you need to be.

Names of "non witnesses". Be surprised how many saw it in hindsight!

Seriously?
Take a pad, pen, some hazard tape and smart phone. The pad and pen are for notes and also to put on the floor or equipment to ascertain size in photographs, the smart phone can do everything else.... send emails, take notes, mark names for witness statements, take the photos, etc and the tape can cordon the area off to give you time to do everything necessary without having the area disturbed. Don't over-complicate it.

Have an idea about the format of the report you are going to make. What I mean is that I have just come back from an incident and I have stared writing the report and realised that I didn’t take a long shot of the piece of equipment involved .. so my report says the piece of equipment is so big , so heavy has four wheels and here is a picture… but all I have is close ups; no overall picture! I have just been back to the scene and they have put the stuff away!
