We already have in place a generic risk assessment relating to the use of MEWP's (Boom Type) but to make it task/site specific I am looking to incorporate the works supervisor/operators documenting their site specific assessments. Does anyone have such form that I can adopt/adapt to meet our needs that they would be willing to share?

Where you identify such a need, I would suggest you incorporate these documents via a staple (or a paper clip).

Not sure if your site teams already have a recorded daily inspection but we have adopted the IPAF inspection below, may be of use.

Thanks guys for what you have provided. I'm sure we have the machine side of things covered as we just adopted the form from the IPAF website what I'm actually looking for is a form for recording the site specific conditions e.g. ground conditions, overhead obstructions, other traffic etc to help turn a generic risk assessment into a site specific one.

Call your generic RA a template and go on from there by populating it with site specific issues & controls.
