Could anyone please share a accident causation checklist with examples

Eg Immediate cause

Underlying/contributory factors

root cause

I have not seen this as a single document

However i have seen the checklist that incorporated to the some companies incident report form, i found it useless as it it banned people to do proper incident investigation and go to tick box option without evidences..
