Does anyone have an audit proforma for assessing compliance re Vibration (HAVS and WBV)? I could produce my own but I think it would have more oomph if I use an independent format, can anyone help? I need to audit our performance in this area.

I think it depends on what you are trying to do. If this is about recording the vibration exposure, there is a 'form' on the HSE website, the Vibration Exposure Calculator that you can use, it is 'amendable', you can add the name of the kit etc and even copy and paste this and do a few changes to make it fit your 'corporate' look.
This doesn't in itself assess compliance but leads you down the road to doing so.
