I'm putting together some PTW Training for the Managers (Non Construction) and I want go through some good case studies, other than Pipa Alpha does anyone have any recommendations of ones worth looking at or ones they have referred to themselves?

Yes, you could assume that everything you read on the internet is true, and that all the marketing material a company produces is absolute gospel. Personally, I value individual recommendations from people that have found a particular example useful and worthwhile higher than the output of marketing departments or whatever appears in google (which is increasingly dominated by the output of marketing departments anyway). The NI HSE reference has some case studies in it, I agree - which ones did you find particularly pertinent to training non-construction managers?

@ Jay & Pikeman Yes they could use a search engine. That is what they are there for after all. They could also use a professional forum to ask for recomendations and advice from fellow professionals. That is what that is there for also.

I found the failings on the Herald of free enterprise disaster pretty good to highlight. Who would have thought it acceptable to sail a ship with the bow doors open??? No disrespect meant

ptaylor, I don't recall failure to implement 'PTW' identified as a root cause in the Herald disaster. Wasn't it lack of communication and procedures?

Evening, I agree with John, Herald of Free enterprise was more of a pre-sea sailing check, so procedural and break down in communication. In the disaster chronicles of the UK I would look at Associated Octel for confined space PTW and HMS Glasgow 1974 for hot work / control of flammable substances. For TV footage (for inclusion to a presentation) visit 'BBC on this day' for the actual coverage. This I have found, definitely injects some realism into the delivery. Hope this is of help.
