Construction phase plan template for small works CDM 2015

Colleagues,
My apologoes if this has been asked before but a search did not throw anything up. I have attended a couple of updates recently on CDM 2015 and both have made reference to a CPP template for smaller works such as kitchen or bathroom replacement on the HSE website but I am unable to find it anywhere.
Would any of my esteemed colleagues out there in H&S land be willing to send me examples of what they use currently or plan to use for smaller works of this nature. In the interests of not reinventing the wheel and using my network to inform best practice I would be extremely grateful.

I too will enjoy seeing that template.
What is smaller works anyway? OK the example given here is bathroom or kitchen but what about replacing a double glazed unit in an existing frame, or attending to replace a ceiling tile and or a piece of carpet?
Would those require a CPP?
One person in eqch of the abouve whearas kitchen or bathroom may require more that one contractor?

I believe the HSE are going to issue one aimed at domestic builder.

As above.
The HSE also provide guidance on what a CPP should be and contained in Draft Guidance L153. Furthermore the aforementioned document states:
The following list of topics should be considered when drawing up the plan:
a) a description of the project such as key dates and details of key members of the project team;
b) the management of the work includes:
the health and safety aims for the project;
the site rules;
arrangements to ensure cooperation between project team members and coordination of their work e.g. regular site meetings;
arrangements for involving workers;
site induction;
welfare facilities; and
fire and emergency procedures;
The above is a relatively short list of topics for a CPP. That said, the challenge for small works is to convert these topics into a proforma which is succinct and practical, which may not be an exact copy of the above list but I suggest keeps within the spirit of the law. Good luck.

Many Thanks for your imput,
I am thinking around developing or amending our existing SWP template which allows for the identification of hazards, PPE requirements, COSHH considerations, Litst the sequence of actions and associated controls etc.
The ammendment would include space to describe what is unique about this environment or property that would not allow the activities to be carried out in the normal way and what actions are being taken to control the alternative activities.
Is my thinking roughly along the lines of my colleagues out there as regards these small works?
We have all in house talent so only ever one contractor and we also would undertake the activities described in Alfaserv's post on very minor repair and maintenance activities.
Can anyone give me some confifdence that I am on the right track or is willing to share learning??

CITB have released a CDM Wizard app, although I can only find it on android at the moment, which has been aimed at the smaller builder. Through a quick process it will produce an action plan which they have classed as The Construction Phase Plan. It might not be exactly what you are looking for but it should give an idea of the content that CITB believe (and I'm guessing they worked with the HSE on this) should be in a small works CPP. Hope it helps

I made one yesterday for one of my client contractors, headings are:
project location etc.
location of emergency services, hospital etc.
Name of first aider on site
Safety goals
Asbestos survey
Description of works
risk assessment prior to staring work inc fire risk assessment
hot works (permit requirements)
fire extinguisher locations and service date
person carrying out the work
a space for any comments
I think it ticks all the boxes?

I think it prudent to wait on HSE publishing their version of a proportionate approach for the small 'domestic' Project. These SHOULD be available early next month.

Thank you all for your responses, they have been greatly appreciated. It is a comfort to know that its not just me feeling a little adrift around proportionallity in this regard. I think I know what I need to do but gettinghte balance right between the requirements and useability is sometimes difficult.
