Does anyone have or can point me to a template for using the Bowtie Investigation technique for accident investigations.

I would appreciate any help you can give

The Netherlands OSH regulator has a very interesting tool called Storybuilder (http://www.rivm.nl/en/Topics/S/Storybuilder). This includes lots of 'standard' bowties for different types of accident, and the data they have about relative frequencies of the different causes.

Orignally developed by consultants, it is now routinely used by Inspectors to aid their on-site investigations into reported accidents
