If you were conducting a very brief staff survey to gauge the health and safety climate of your firm, what would your top questions be? Say between 3 and 10 questions max?

..whilst not allowing you to read into the safety climate too much - I always like to ask for the definition of a hazard or risk - I'm always surprised at the lack of knowledge with regards the distinction between the two.

Having seen the results of a number of these surveys over the past 20 years my cynical view is this is what it breaks down to:
The workforce will say that H&S is not a management priority
The managers will say failures are caused by the workforce not doing as they are told.
When the results are presented to the management the whole thing gets quietly dropped.

As a rule safety climate surveys are broken down into a few domains, such as - communications, supervision, risk taking, welfare, health, senior management commitment, etc. With only a sparse amount of questions or statements you may as well pick one of each. Clearly with such a limited survey you will only get a precis of the safety culture.
The actual questions or statements (I prefer statements) need to focus on the type of company and activities. For example, if working in a high risk industry you might like to ask 'We often take risks in order to get the job done'. A three or five point Likert scale is the norm for responses. All surveys should be confidential with only very limited demographics.
