Interesting reading but I wish they would give more details:- http://press.hse.gov.uk/...lease&cr=14-Apr-2016 Not sure what to make of this!. Was the risk assessment shocking poor or was it that they failed to follow up? I suspect the following which should be warning to all consultants.

Can't see it being the consultant "that didn't follow up" - they may have been paid only for the RA, not for following up, should you do this unpaid? It sounds more likely that it was not "sufficient and suitable". My experience is that clients often want a RA to rubber stamp what they are doing, rather than "tell it like it is". The problem is, you tell clients what they don't want to hear, so they get someone else who rolls over...............

The inadequate training and supervision, identified in the report, should have been picked up in the risk assessment for starters. It looks as though the RA not carried out properly: "The risk assessments and procedures provided by Mr Baker were neither suitable nor sufficient to control risks arising from the operation of these two machines". Perhaps the consultant did not know enough, or did not consult with the operatives...

It's the consultancy that was prosecuted, not the consultants.

The problem is there are just too many rubbish RAs which are generically orientated. Nothing really ever comes of it in terms of enforcement until that is 'crunch'. Until this blaise attitude changes there will be a reliance on poor documentation which is not fit for purpose.

No mention of breach of Management Regs 7(1) - unless that was plea bargained out. Whither competency............................?

No sympathy whatsoever with the consultancies / consultants, should have known better. Me/my company never ever did a work/workplace RA....... facilitate the process yes, but down to the workers/company to do the actual RA. This approach is the preferred route by the HSE.

Similar situation, I had a client a few years back that wanted me to 'do' the Risk Assessments, when I recommended that I can train a person on site and assist them with the help with other operatives etc, they didn't want to know. Needless to say they didn't remain a client for long as they just wanted the paperwork to put in a file.

That approach to the problem of many people is surprising. But such http://szkoleniebhp.com.pl/ training is safety for them and for others..
