Hi All I am after a simple prequalification format that we can use when we employ contractors. Would any of you either have an example you would be willing to share or could point me in the direction of some examples online? Thanks

PAS:91 is for Construction prequalification questionnaires, please remember that when you send it to company's who are not in construction, they may not be able to answer construction specific questions. Give them a get out if that is not what they do - please Also don't ask questions for the sake of it, if you don't really care about this or that, don't ask just because you think you should. We actually had one customer who wanted something stupid and we turned around and said ok we will not come on your site. If you want it sorted bring it to us - sometimes you can have a good day.
