Hello, i have just injured my back at work and have been told to register the accident by filling in a DSS Injury At Work Form by a friend, in case i suffer from the incident in later life etc.I have scoured the internet for such a form but cannot find anything. I was just hoping to find someone who could point me ith the right direction. Thankyou.

Your friend is thinking of the B510 which is also referred to as the Accident Book. You won't find it on line as it is a document your employer is required to keep.
Ideally basic details should have been recorded before you left work.
This can be completed when you return (but if you search the forum you will see this causes issues for the employer as it delays investigation and may result in someone else suffering a similar injury).
If you are likely to miss the next few days whilst recovering email or write to your employer giving as much detail as possible - date, time, where you were, what you were doing and if anyone else saw what happened.

Hello, thankyou for the reply and information. I reported the accident at work and they took all the information down and entered it into the accident book which i then signed.Should that be enough then? Is it wise to report this anywhere else? Thankyou.
