I am looking for an example or blank template of an HSE improvement form but so far have been unable to find anything.
Can anybody point me in the right direction please?

I don't have one myself but you can buy blank forms from Shaw & Sons, though I think they only sell them in bundles. I don't know if they will sell them to you if you're not an Enforcement bod but you could ring and ask - 01322 621100.

Searching using a well known search engine using the terms
hse improvement notice +"LP1"
comes up with 3 pdf examples of what appear to be actual INs.
The HSE website enforcement guide pages explain the notice procedure in detail.

The Improvement notice LPS1/LP1 is availble, as noted above, but what matters is what's written on it, or perhaps more accurately, what's written on the schedule attached to it, as that will set out what needs to be done ("or any other equally effective measures to address the breach")
The Schedule is the bit that takes the time when drafting an IN. For some common breaches there were templates, which used to be available on-line via HSE, but I generally prefered to draft my own tailored to the case in question.
Also important to establish the scope of the notice - one machine, one site or across the whole business?
A well written IN will help the recipient comply by being clear, concise, and structured. It should also have clear conclusion, a point at which compliance can be measured, not be open ended - ongoing compliance is the job of the act /regulations
