On reviewing web based H & S managements systems I came across incident reporting module with a 'Main Cause' field and other field for 'Type of Incident'.

Can anyone enlighten me on the significant differences on the type of information that would belong in each of these 'fields' as I am struggling to differentiate between the two.

If I am reading this right, I would say 'Type of incident' e.g. fall from height. Therfore 'main cause' e.g. faulty ladder

I would agree with WatsonD, you could say that "Type of incident" could also mean is it a first aid case, medical treatment case, fatality etc... but I think I'd go along the lines of WatsonD's suggestion.
