Hi, my first post since joining IOSH.I work as a risk management and safety officer. I am new to this role. There are lots of incidents to be investigated. I am searching for a template to document investigation findings in order to report to senior management team. Any advice would be greatly appreciated😊

Hi Chrissie Normally you would undertake an investigation report of some description depending on the severity on the accident/incident. Within the report there is usually an executive summary at the beginning for senior management to peruse because they are so busy they don't have the time to read the whole report. You could of course populate a spreadsheet if you have a lot of incidents with just the title and main cause(s) if you wish. There are few hard and fast rules.
