If your course was registered with IOSH then they should be able to help.

I found this a tough one, we had a new start who had completed an IOSH Managing Safely course with his previous employer but he didn't get his certificate when he left and we weren't able to verify if he had completed the course or not! he had to contact IOSH directly and get verification but I don't think he got this

Point to note: It will cost £30 for a certificate re-print from IOSH.

I think that with most people going on to do Nebosh, NVQs and NCRQ worrying about the managing safely and if it runs out is silly as your just going backwards, I did mine in 2015 with the BSC to be honest i'm not overly worried if it lapses or not. I used it to get my black cscs skillcard and it has a valid to date as 2018.

As its been said its good to keep having refreshers as things do change, but if your doing other courses its pointless going back and doing that just for the sake in having it.

Perhaps getting a replacement without the date might be worth the £30 but i'd sonner spend that in forwarding my career as opposed to hanging on to the past
