Can anyone post or provide a report on a real life recent accident which caused injury (obviously avoiding sensitive data such as names and addressess). If possible something that was not a huge event and already well covered e.g. Kegworth. Herald of Free Enterpise, Tenby, etc

Im interested in examples of a recorded re-active event which looks towards contributory factors such as task, environment, equipment or chemicals and examines cause, findings and conclusions. And from a root cause analysis a plan of action that significantlly reduced the lilihood of a re-occurence,

Have a look on the HSE website for their Incident reports (under FOIs) - the F1 test driver crash involving Maria De Villota is one of the more recent investigations - certainly highlights a few issues, albeit from a regulatory/enforcement perspective. http://www.hse.gov.uk/fo...-maria-%20de-villota.pdf Jamie
