May have been discussed before - apologies - but looking for some thoughts on this one. FLT driver is loading brick packs with a 4 fork FLT. During stacking operation he encounters a large pot hole disguised by being full of water - the FLT overturns - with thankfully minor bump to driver and minimal damage to truck and product.

There is a distinct difference between overturn and fall over,  The first is reportable as a DO but the latter is not.  If the truck does not rollover onto the roof or further it is a fall over which according to a strict reading of the regs is not reportable

Yes, it is reportable to the HSE under RIDDOR as a dangerous occurrence.

Dangerous occurrences are certain, specified ‘near-miss’ events (incidents with the potential to cause harm.) This could have been the fork lift truck (FLT) operator or anyone in the vicinity who was unlucky enough to be in the way when the FLT “overturns”.

Schedule 2 of RIDDOR Dangerous Occurrences, lifting equipment states: “The overturning of any lifting equipment.” E.g. a FLT.

Given that the HSE advise the following:

  • FLT travel not to take place on uneven ground unless the lift truck is suitable for this e.g. a rough terrain FLT
  • A vehicle must not be used in a way that might cause it to overturn, including being driven over unsuitable ground e.g. potholes
  • potholes should not be allowed to develop, and if they are found they should be repaired promptly

Due to the above and the information you provided this incident would be classed as a dangerous occurrence and would therefore be reportable under RIDDOR.

Many thanks guys - I did wonder how a FLT would roll over onto its roof? Much discussion has been had with a fellow CMIOSH - we often beg to differ - but it is healthy conversation - keep up the good work!!
