We have an employee who has been unofficially seconded out from a limited liability partnership company to one of the partnerships parent company during this secondment the operative was carrying out work for a third party on behalf of the parent company when he had an accident a fall and fractured his leg who is responsible for completing the RIDDOR report

WOW... that's confusing.

RIDDOR reports should be completed by a responsible person, in this case the employer should make the report if they have been unofficially seconded then the limited liability partnership company is responsible.

Regardless of who's responsible a joint investigation should be undertaken by the companies involved.

I hope the individual has a speedy recovery.
