Re Reg 5 of CWAR. I am looking for a simple template that will allow me to carry out a preliminary noise assessment (not survey) to determine the next stage. I have got lots of information from the HSE website with tables and formulae etc, but cannot find an assessment form anywhere with which to get started.Can anybody point me in the direction of such a document? Ta.

I find that the template used for the generic MHSW regs we use is a suitable template to use for many other areas; so use the one U use for MHSW as the one for noise in the first instance

Once you have the survey and have calculated the equivalent daily exposure levels from your results you can actually do the next stage just in a simple table. Headers would be something like - sample number, area in which taken, calculated daily exposure in dB(A), classification under CNWR (i.e. below lower exposure action value, between lower and upper or exceeds upper), whether the exposure limit value is exceeded, what the main noise source is and whether action is to be taken. Then do a separate table for the action plan saying what is to be done, when and by whom. Leave a column to mark when the action is completed. Obviously if this is done for a client, the whole thing is wrapped up in a report that explains the methodology, the legal requirements and so on.
