I work in the events industry and the use of FLT & Telihandlers with man cages, (non integrated) is common practice. I am aware of PM28 and the strategic review, not quite certain where the advice lies now, as this is not a construction and the use of MEWP introduces more hazards than it removes.
I have suggested, SSOW, 6 month LOLER, Fall restraint harness, and regular inspection of cage etc, agreed comms between drive and person in the cage and driver to have appropriate FLT certificates of competency.

We currently run a fleet of telehandlers on construction sites and will only have man cages on site if all other means are exhausted, as the guidance note stipulates variable reach trucks (telehandlers) cannot have a reach of above 6mtrs if one is attached I don't think this just applies to construction. If we were to use one it would have to go through the correct planning process with all operatives and personnel trained (CPCS or better) in use of attachment and truck also no more than 1200 restraint harness with training on use and inspection.
6 months LOLER on the attachment and the truck daily checksheets with failsafe checked tilt mechanism locked sufficient communication this goes on and on, PM28 is a guidance from HSE as good as or better so be careful when using.

I think you answered your own question with this being 'common practice', use of cages and platforms on FLT's and the such like are supposed to be infrequent (well, PM28 points to that anyway), least preferred options, even with locks, anti-tilt mechanisms and fall restraint etc. There are other integrated platforms besides MEWP's that could be hired or bought that would be much more appropriate, good for rough ground, need fewer people, can be used indoors or are transportable. If its just once in a blue moon then just use a FLT, absolutely it must be locked to the forks, and have a physical barrier to tilting as well as the other good things you mention.
If its 'common practice' then I'd suggest an alternative......

Agree with Corfield about alternatives however if you press on with cages (your perogitive) i would also suggest pre use operator checklist of salient parts of the cage/flt interface and any other safety related item/system.
