Can anybody help???

I want to complete some training needs analysis, can anybody point in the direction of some standard templates / questionnaires that could help me as a starting point.

If anybody has completed a training needs analysis in the retail and distribution sector your input would be ideal.

'Training in Organizations', by I L Goldstein & J K Ford, 4th edition, Wadsworth 2002, is a well-researched guide to this in the context of the whole cylce of training, learning and development
